Once you have made this important decision for yourself, it can be important to feel that everything gets started fairly quickly. It is important to me that you feel comfortable in moving forward in working together, and beginning therapy can feel overwhelming, so communication is key to helping us get started.
An easy way to make a connection is through email -email@example.com. Let me know briefly, with information you are comfortable sharing, that you are interested in working together. Once we have mutually agreed to set an appointment time together, I will send you an email with a link to my client portal that contains questionnaires to be completed, as well as additional documents about my practice.
If contacting me by phone is more comfortable for you, please do! You can reach me at 706-621-3024. You will always have the opportunity to let me know what you are looking for in therapy, and ask me questions about my practice. I will also ask you a few questions to better understand your needs, as well as general information. Again, once we have mutually agreed on an initial appointment time, you will receive an email with a link to the client portal. Forms need to be completed 48 hours to keep the reserved appointment time.

Intake Questionnaire (History, reasons for seeking therapy, etc)
Informed Consent for Treatment
Practice Policies and Service Agreement
Informed Consent for Telehealth Psychotherapy (as needed)
Additional questionnaires/documents may be included as appropriate.
